Description of Annuity Forms

Life Annuity - No Death Benefit - Provides lifetime monthly annuity payments to the Participant with no payments after death.

Certain & Continuous Annuity (C&C Annuity) - Provides lifetime monthly annuity payments to the Participant. In the event the Participant dies within the designated certain period following the date annuity payments begin, payments in the same amount will continue to be paid to the beneficiary designated by the Participant for the remainder of the certain period.

Period Certain Annuity - Provides a monthly annuity payment to the participant for the designated certain period only. In the event the Participant dies within the designated certain period following the date annuity payments begin, payments in the same amount will continue to be paid to the beneficiary designated by the Participant for the remainder of the certain period.

Joint & Survivor Annuity (True J&S) - Provides lifetime monthly annuity payments to the Participant. Upon the death of either the Participant or the Joint Annuitant, the amount of payments continued to the survivor will depend on the election made by the Participant at retirement. It may be 100%, 75%, 66 2/3% or 50% of the amount of the payments made to the Participant before death.

Contingent Annuitant Option - Provides lifetime monthly annuity payments to the Participant. Upon the Participant's death, the Contingent Annuitant designated by the Participant, if living, will receive monthly payments for life. The amount of payments continued to the Contingent Annuitant will depend on the election made by the Participant at retirement. It may be 100%, 75%, 66 2/3%, or 50% of the amount of the payments made to the Participant before death.

Full Cash Refund Annuity - Provides lifetime monthly annuity payments to the Participant and includes a provision for a death benefit payable to designated beneficiaries. The amount of this death benefit will be equal to the excess, if any, of the purchase price of the annuity over the total of the annuity payments made to the Participant.
